SUBJECT: Acceptance of the Renewal Rate for the Voluntary Dental Plan
Personnel Manager Randy Peck is recommending City Council approval of the renewal
rates for the Voluntary Dental Plan, which is paid in full by the employees. The single
premium rose from $26.34 per month to $28.32 per month, and the family plan rose
from $79.01 per month to $84.93 per month.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

SUBJECT: Acceptance of the Renewal Rate for the Voluntary Dental Plan
The premium for the dental plan, which is paid in full by the employee increased from
$26.34 per month to $28.32 per month for the single plan and from $79.01 to $84.93 per
month for the family plan. These rates are in effect from January 1, 2005 through
December 31,2005. I recommend that the renewal rates be accepted. The requested
action is for the City Council to approve the renewal rates and authorize you to sign the
attached financial exhibit.
