Re:
Claim of Ms." Florence Hird
Claimant
Date of Claim. Date of Loss Nature of Claim.
Florence Hird
11/9/00 . 10/2/00 property damage
This is a claim for damages to the claimant's basement door,
which it sustained when the Dubuque police made a forced entry to
do a welfare check on the claimant.
According to the report of Acting Police Chief Terry Lambert,
neighbors were concerned about the claimant because they had not
seen her for some time, her mail was piling up and her car was in
the garage. An 'officer was not able to get a response at the
house, and he forced entry to the basement, causing minimal damage
to a door. Acting Chief Lambert recommended payment of the claim.
It is therefore the recommendation of the Legal Department
that this claim, in the amount of $59.15, be paid by the Finance
Director and submitted to ICAP for reimbursement.

Dear Ms. Gulick:
On October 2, 2000 the Police Department received a call from neighbors
of Ms. Florence Hird who resides at 1110 Wooten Street. They reported that
they had not seen Ms. Hird for some time, her mail was piling up and her car
was in the garage.
Officer Kurt Horch responded to the residence and when he was not able to
get a response at the door her made a forced entry to check on the welfare of
Ms. Hird. It was later learned that she was located at Mercy hospital where
she was recovering from surgery.
The etimated damage to Ms. Hird's residence as reported is minimal and I
would recommend we pay it.
Sincerely,
&:y~~
Terry Lambert
