Since its inception in October 2006, Crescent has seen tremendous growth and now serves over 6,000 patients for over 15,000 visits per year. Because of the increased need in the community, the Immediate Facility Improvement Grant funds are needed to: 1. Expand the waiting room to accommodate the increasing numbers of patients and their families; 2. Finish a portion of the basement for storage of paper medical records; 3. Purchase additional dental supplies to improve patient flow through the dental clinic; 4. Expand to provide an office area for staff and patients for care coordination services which are a part of the patient centered medical home requirements; and 5. Provide baby changing equipment for both the men's and women's bathrooms. Crescent Community Health Center is a vital part to our neighborhoods, the city of Dubuque and the region. Crescent has shown its commitment to the community and it is an organization capable of administering high - quality care to its patients while also planning for the future and remaining fiscally sound.
