This memorandum is to seek your approval and execution of Amendment No. 5 to the July 19, 2010
Agreement for Construction - Related Services between the City of Dubuque and Strand Associates,
Inc. (Strand) for the Water Pollution Control Plant (WPCP) Modifications Project (Project).
DISCUSSION
Amendment No. 5 provides for approximately 130 hours of additional onsite inspection time as it
relates to the July 11, 2011 digester reinforcing steel collapse and the July 28, 2011 site flooding
repairs.
BUDGET IMPACT
Amendment No. 5 represents a $20,000 increase (from $30,000 to $50,000) for this work. It will be
funded by the contingency funds of WPC Plant Facility, Capital Improvement Project (CIP 7101837),
leaving a contingency balance of $1,026,060.93.
ACTION TO BE TAKEN
The execution of Amendment No. 5 to the July 19, 2010 Strand Associates agreement is requested.

This is Amendment No. 5 to the referenced Agreement.
Under Scope of Services, Insurance Claims- Related Services, ADD the following:
"1 Provide approximately 130 hours of additional RPR time required because of the

July 28, 2011."
Under Compensation, in the second table, Insurance Claims- Related Services, CHANGE
$30,000 to "$50,000."
