This is a claim in which claimant alleges that a City water main collapsed during the July
28, 2011 rain event, causing water damage to claimant's property at 11735 King Arthur
Court.
This claim has been referred to Public Entity Risk Services of Iowa, the agent for the Iowa
Communities Assurance Pool.

CLAIM AGAINST THE CITY OF DUBUQUE, IOWA
This written report constitutes your claim against the City of Dubuque, Iowa. You
should complete this form in full and attach any additional information that
supports your claim.
The claim must be filed with the City Clerk at City Hall, 50 West 13th St.,
Dubuque, IA 52001. It will then be referred to the appropriate department for
investigation and to the City Attorney's Office. Once that investigation is
completed, a report and recommendation will be submitted to the City Council.
You will be provided with a copy of that report and recommendation.
The final decision on all claims is made by the City Council. No employee of the
City of Dubuque has the authority to make any representation to you as to
whether your claim will or will not be paid.

To: City of Dubuque
To whom it may concern:
The major rainfall of July 28, 2011 caused significant damage to my property located at
523 Kaufmann Avenue, which was occupied at the time of the storm. There were
massive amounts of water coming up out of the floor drains, as well as damage to the
personal property of the renters and to the home itself.
It was necessary to remove the damaged materials and repair and replace with new, so
that the home was inhabitable for the renters.
I have enclosed receipts for all of the work that was done as a result of the water
damage from the storm.
I had remodeled the house a year prior to the storm at a cost of approximately $5,000.
My accountant has receipts in which I can supply if needed. I mention this so that you
are aware that the damage done was not to an old, worn out or neglected home, but to
a newly renovated home.
There was also major damage to the city system at the bottom of Hempstead Street,
which is very close to my property at 523 Kaufmann Avenue. The man hole cover was
off and not in place, and the street collapsed, which took several days to repair.
I had never had any problems with water until this major occurrence on July 28, 2011.
