Dubuque On the Move - C. Sinsky MD July 7, 2005 Honorable Mayor and City Council c/o City Clerk City Hall 50 W. 13th Street Dubuque, IA 52001 Re: Dubuque on the Move Letter of Support Dubuque on the Move is a community coalition of partners from the public and private sector working together to make Dubuque a healthier place to live. We are applying for funding through Building Healthy Communities in Iowa Harkin Wellness Grants, and are requesting a letter of support from the Dubuque City Council to accompany our grant application. The Harkin Wellness Grants will support 36 initiatives across Iowa at levels of $75,000 to $250,000 that: "Create coordinated programs at the community level to encourage healthier lifestyles, promote wellness and prevent the healthy and financial consequences of chronic disease that are often lifestyle related... Applicants, in a collaborative partnership with other entities within the community, will plan, implement, evaluate, and institutionalize established or innovative strategies that contribute directly to the prevention, delay and/or mitigation of preventable premature chronic disease, addressing good nutrition, increased physical activity, reduced tobacco use, and promotion of positive well being." Current participants in Dubuque on the Move include the Dubuque Community Schools, Visiting Nurses Association, Project Concern, Tri-State Occupational Health, Western Dubuque Schools, Medical Associates Clinic, Dubuque County Board of Health, the City public health nurse, Mississippi Valley Running Association, and Dubuque Health and Fitness, Inc. Enthusiasm and synergy has been terrific, and participation is expected to continue to grow. Attached is further introduction is our mission, organization, and plans for Phase 1 programmmg. A coalition has been meeting for several months to address wellness activities in the Dubuque community. Public Health Specialist Mary Rose Corrigan has been involved in this process to provide the appropriate public health data and linkages to the project. Leisure Services Manager Gil Spence is also involved in the planning and implementation of specified walk/run areas in the community, which will be provided with signage. The City's Public Works Department will donate the installation of the Dubuque on the Move designated walking/running path signs and mileage markers at an estimated value of $3,650. The Dubuque community has the infrastructure through both City government and community-based organizations and agencies to address smoking, weight, nutrition and exercise through the Dubuque on the Move program. Through the efforts of the City's Health Services Department and Leisure Services Department and their participation in the Dubuque on the Move coalition, a successful project can be implemented. Sinsky, M.D.) Introduction Dubuque on the Move is a community collaborative of partners from both the public and private sectors, established to create a sustainable "culture of health" in Dubuque County. The partnership shares a vision of broad-based action, focused on identified community needs for healthy lifestyle promotion, to be achieved through outcomes-based advocacy, well- integrated community linkages, and a clear, consistent message. The four characteristics of a healthy lifestyle (weight, nutrition, exercise, and non-smoking), will be targeted at multiple levels within the community. We share the understanding that health promotion occurs with a change in the entire ecology of a community. Lifestyle accounts for 40% of chronic diseases, yet only 3% of Americans achieve all four healthy lifestyle characteristics. The relationship oflifestyle to physical health, mental well- being and to healthcare costs makes this a priority for individual citizens, health care providers, and businesses that purchase healthcare for their employees. Goal The goal of Dubuque on the Move is to continue to build our coalition of stakeholders among health care, social services, community members, business, and government, to create an environment that supports healthy lifestyles at every level. This is a comprehensive effort, aimed at infrastructure development, immediate evidenced-based programming, long-term policy strategies, and ongoing fund development for sustainability. The scope is broad, highly inclusive, and designed for both cultural and demographic diversity. Infrastructure Infrastructure development will entail establishing an overarching structure for orchestration of wide community participation, coordination of linked, focused interventions, and facilitation of ongoing, creative responses to community health promotion needs. Dubuque on the Move infrastructure will include the executive director, board of directors, project coordinator, fund development coordinator, interactive web page, video and print materials, organizing tools, and a data center. This core structure will maintain a single, visible, consistent message in each project, direct measurement of concrete outcomes, facilitate cross-linkage among discrete programming elements, and provide the communication matrix wherein creativity and synergy flourish. A strong, sustainable infrastructure is critical for the infusion of Dubuque on the Move into all levels of the community, and is essential to the goal of developing an overall "culture of health" within the community. Page 1 of3 Phase 1 Pro2rammin2 There will be a consistent message, focused on the four healthy lifestyle characteristics, interaction between each of the programs (for example, those encountered through the school or business programs will be offered referral for weight management and smoking cessation counseling when appropriate), an integrated approach to Dubuque on the Move promotion (for example, on the web page, and cross promotion of each element at the other programs), and a coordinated approach to measurement (for example, while there will be program-specific measurements, there is also a standard set of healthy lifestyle characteristic measures that will be obtained within each element.) 1. Dubuque Walks-- Dubuque on the Move Establish neighborhood walking routes with mileage markers, sidewalk stamps and Dubuque on the Move signage. City Public Health, Mary Rose Corrigan RN 2. The Business Case for Health Promotion-- Dubuque on the Move The goal is a shift in the working environment to support healthy lifestyles. Business can incent healthy lifestyles among their employees by "take a walking break" incentives, giving an extra 15-minute break to those who go for a walk during lunchlbreak; by providing healthy vending machine snacks and drinks; and/or by pledging to choose healthy food at a business sponsored dinner. Health promotion modules (speakers, brochures, referrals for weight management and smoking cessation) could be imported into the business setting when appropriate to the company. A community wide campaign for multiple businesses to go smoke-free on their campuses simultaneously will generate interest and support. Page 2 of3 Measurement Data will be collected regarding the efficacy of each structured intervention and shared in both public and professional literature so that other communities can review this model and adopt it to their own needs. Participant, process, and outcomes data will also be collected to assess the efficacy of the overall intervention. Concrete measurable outcomes and ongoing evaluation will be used to generate momentum from early success, garner on-going financial support, refine Phase II programming interventions, and share with other communities for widespread health promotion.
