This memorandum is to request the execution of a Additional Service Request for
continued environmental consulting services between MWH America's, Inc. (MHW)
and the City for the Water & Resource Recovery Center (W &RRC) Modifications
Project. This proposal request relates to the continued environmental engineering
services necessary for conducting a Risk -Based Corrective Action investigation and
to complete a Tier 2 Site Clean Up Report related to the former petroleum product
underground storage tank (UST) at the W &RRC site.
DISCUSSION
This contract amendment request in the amount of $26,550 provides for the
following UST clean -up activities:
• Fieldwork involving the installation of four groundwater monitoring wells, and
a soil vapor monitoring well. Soil and groundwater samples will be collected
for laboratory analysis. A receptor study will also be conducted to locate
nearby receptors that could be at risk from the petroleum hydrocarbon
release.
• Reporting — MWH will compile a Tier 2 Site Cleanup Report using the newly
collected data. Once the report is completed it will be submitted to IDNR for
final approval.
BUDGET IMPACT
This fifth request brings the contract total to $199,200. The increase will be paid from
the contingency funds of the construction project.
ACTION TO BE TAKEN
The execution of a budget increase request for continued environmental consulting
services with MWH Americas, Inc. in the amount of $26,550 is requested.

Dear Mr. Sampson Brown:
MWH Americas, Inc. (MWH) appreciates the opportunity to submit this proposal to conduct a
Risk -Based Corrective Action (RBCA) investigation and complete a Tier 2 Site Cleanup Report
(SCR) for the City of Dubuque, Iowa (City) related to the former petroleum product underground
storage tank (UST) at the City's water pollution control plant (WPCP) site. This proposal is
intended to satisfy the Iowa Department of Natural Resources (IDNR) requirements, as outlined
in their February 4, 2011 letter. MWH understands the Tier 2 SCR Report is due to the IDNR by
May 31, 2013. The proposed activities are intended to compliment the previously collected data
as outlined below.
Task 1 — Fieldwork
MWH will contract with and supervise an Iowa - licensed drilling company to advance, continuous
sample, and install two 2 -inch diameter polyvinyl chloride (PVC) groundwater monitoring wells,
and one 1 -inch diameter PVC soil vapor monitoring well hydraulically downgradient of the
existing monitoring wells in an attempt to delineate the extent of petroleum hydrocarbons
associated with the former UST. The groundwater monitoring wells will be installed at an
approximate depth of 35 feet below grade, as determined in the field, and the soil vapor well will
be installed at an approximate depth of 14 feet below grade.
All soil samples collected for laboratory analysis will be placed in pre - cleaned 4 -ounce glass jars
and stored in an iced cooler. The samples will be delivered under chain -of- custody (COC)
documentation to Keystone Laboratories, Inc. (Keystone) in Newton, Iowa for analysis of
benzene, toluene, ethylbenzene, and xylenes (BTEX) and methyl tertiary butyl ether (MTBE) by
Iowa Method OA -1.

Existing and newly installed wells will be measured and surveyed using a level and rod to obtain
horizontal control and vertical elevation control. Soil cuttings recovered from drilling activities
are planned to be thin - spread on site in a location approved by the client.
Existing and newly - installed groundwater monitoring wells will be gauged to measure the static
water level using an electronic interface probe. A groundwater flow direction map will be
constructed based on the water table data. Monitoring wells will be purged and sampled using
disposable bailers. Groundwater samples will be placed in 40- mililiter glass vials preserved with
hydrochloric acid and stored in an iced cooler. The samples will be delivered under COC to
Keystone for analysis of benzene, toluene, ethylbenzene, and xylenes (BTEX); and methyl tert-
butyl ether (MTBE) by Iowa Method OA -1.
A second mobilization event is budgeted with installation of two additional groundwater
monitoring wells should laboratory results indicate further plume delineation is needed. The
additional wells and mobilization will not be used unless they are necessary.
A receptor survey will also be conducted to locate nearby receptors that could be a risk from the
petroleum hydrocarbon release. Receptors include sanitary and potable water lines,
basements, and drinking or non - drinking water wells. As much information as possible
regarding the receptors will be obtained from previous site investigation activities, City records,
and other sources before mobilizing to the site.
Task 2 — Reporting
MWH will compile a Tier 2 Site Cleanup Report (SCR) using the existing and newly collected
data. Additional information will be collected from the State of Iowa and City offices, as
necessary. The Tier 2 SCR will be completed under the direction of an Iowa - Certified
Groundwater Professional (CGP) and sent to the City for review and signature. Upon receiving
approval and signature, the Tier 2 SCR will be submitted to the IDNR.
Task 3 — Project Management
This subtask includes project management time for project administration; communication with
City representatives, subcontractors, MWH staff, and IDNR staff; and associated activities.
ESTIMATED COSTS
The tasks will be completed on a time - and - materials basis in accordance with the October 18,
2010 Consulting Services Agreement (CSA) and MWH's current "Fee Schedule," attached. The

estimated cost to complete the tasks identified above is approximately $26,550, as indicated in
the following table:

• Subcontracted services will be invoiced at actual cost plus fifteen (15) percent for general and
administrative expenses.
• Other Out -of- Pocket Expenses, including travel expenses and shipping expenses (UPS /FedEx for
reports, materials, supplies, and laboratory samples), will be invoiced at actual cost plus twelve (12)
percent for general and administrative expenses.
• Health & Safety, Field and Construction Equipment will be invoiced according to MWH AMERICAS,
INC.'s standard unit rates. A partial list is available upon request.
• Leased and personal vehicle usage will be invoiced as follows (rental vehicles will be charged at
actual cost plus twelve [12] percent):

Field Vehicle $ .65 per mile plus $35.00 per day*
* Adjusted in conjunction with the federal allowable rate.
• Associated project expenses, including telecommunications (equipment, local and long distance
charges), facsimile, computer and word processing equipment, first class postage, network charges,
and in -house reproduction and printing of plans, reports, and other documents (black & white, up to
11x17 inches, up to 5 copies) will be invoiced at $10.40 per total labor hour, in lieu of detailed
invoicing of the specified items.
• A surcharge of fifty (50) percent will be added to hourly rates for expert witness testimony,
participation, preparation time, hearings, depositions, etc.
• Rates for Computer -Aided Design and Drafting equipment, when required, will be invoiced at the rate
of $15.00 per hour.
• A surcharge for Contractors Pollution Liability Insurance coverage on Remediation /Construction
activities will be negotiated for each contract where required.
• The above rates are subject to change January 1 of each year.
• A service fee will be added to the unpaid balance after 30 days at the rate of eighteen (18) percent
per year.
