Dear Mayor Buol:
Thank you for the timely submittal of your Quarterly Report for the period of October
through December, 2012. This letter serves as the Department's assessment of your performance in
implementing your approved Healthy Homes Demonstration work plan activities during this period.
Based upon information submitted in the quarterly report, your program has achieved an overall
performance rating of 100 out of 100.
The City's program has received a Green designation, meaning the program is meeting
and/or exceeding its the primary benchmarks. Congratulations on meeting or exceeding the
remaining benchmarks /milestones, and not only addressing the health hazards, but also making so
many healthier housing units affordable.
Enclosed is a complete report providing details regarding the strengths, weaknesses, and

staff are keeping the program on track and focused on the target population and high risk housing in
your community for lead poisoning. Congratulations on filling your Healthy Home specialist
position. The Race and Ethnicity form has been received and filed for your program. Your
registration for program manager's school has been received. Thank you!
Corrective Actions: Please ensure that the monitoring visit report response is submitted
immediately it is overdue. Continue to work with your GTR to provide guidance and refocus with
the issues noted in the report addressed as we move forward.

Dear Mayor Buol:
Thank you for the timely submittal of your Quarterly Report for the period of October
through December, 2012. HUD's Office of Healthy Homes and Lead Hazard Control (OHHLHC)
completed an assessment of your overall performance based on submitted reports and the grant
work plan. HUD assesses program performance based on several factors including 1) progress in
meeting established benchmarks such as completing lead hazard control in residential units and 2)
progress in drawing down grant funds relative to the work performed. Based upon your quarterly
report your program has an overall performance rating of 100 out of 100.
Your performance is indicative of excellent progress towards goals. Grant program staff has
performed superbly in meeting contract benchmarks. However, the OHHLHC has not received
your response to the monitoring report sent on November 7, 2012. Please submit your response
immediately.
