The City of Dubuque was awarded a Fiscal Year 2007 Shelter Plus Care Grant in the
amount of $536,520.00 for Phoenix Housing Shelter Plus Care Project for disabled
homeless households. This five year grant expired June 10, 2012.
Housing and Community Development Director Alvin Nash recommends City Council
authorization for the Mayor to execute documents to close out the Fiscal Year 2007
Shelter Plus Care Five -Year Grant.

INTRODUCTION
This memorandum requests City Council authorizing the Mayor to sign documents to
close out the FY 2007 Shelter Plus Care Grant (SPC) Five -Year Grant.
DISCUSSION
The City of Dubuque was awarded a Fiscal Year 2007 Shelter Plus Care Grant in the
amount of $536,520.00 for Phoenix Housing Shelter Plus Care Project for disabled
homeless households. Grant Number IA26C601021 was for 5 years beginning June
11, 2007. The City of Dubuque then contracted with Project Concern for Sponsor -
Based Rental Assistance (SRA) in July 2007. The grant has expired as of June 10,
2012; therefore, we are processing the grant for closeout.
The SPC program provided rental assistance to homeless and disabled individuals and
families in the amount of $225,704.00. Project Concern provided supportive services in
an amount at least equal to the rental assistance during the term of the grant to assist 34
households.
A final monitoring was performed by HUD staff on July 10 -11, 2012. In response to one
finding, it was noted that the project sponsor, Project Concern, had submitted draw
requests to the City of Dubuque that were not adequately documented. The City has
requested a refund from Project Concern in the amount of $12,493.
A grant closeout certification must now be executed by the City to proceed with the
SPC grant closeout. The City Council must approve execution of the certification to
facilitate submittal of closeout documentation to HUD.
RECOMMENDATION
I request that the City Manager recommend City Council approval of the attached
Continuum of Care Grantee Closeout Certification authorizing grant IA26C601021
Phoenix Housing Shelter Plus Care Program be closed. This will allow the City to
submit necessary documentation to HUD to close the five -year grant.

such funds are subject to coverage in the next single audit. The grantee may be required to repay
HUD any disallowed costs based on the results of the audit.
Enclosed are three copies of the Continuum of Care Grantee Closeout Certification.
Please execute all three copies and return two of the certifications to this office. The remaining
copy is for your records. Also, please provide an inventory of any real property and equipment
purchased with grant funds.
