Dubuque County Auditor Denise Dolan initiated a proposal for a telecommunications
carrier assessment and study in 2012 for Dubuque County and granted the contract to
LINX Solutions. The board approved her recommendation in the fall of 2012. The
auditor has invited the City to be a part of the study as we were in 2007.
By issuing a joint proposal it is anticipated that not only cost savings but also
opportunities for telecommunications contingency and fail over could be gained for both
the City and County. It should also be noted that the city and county are under no
obligation to choose a solution that would obligate either party to a joint solution or a
support scenario that is not in the best interest of the individual entities. In other words
although the city and county are issuing a "joint proposal" we're not looking for a
"joint /shared solution" unless there are benefits agreeable to all parties.
Approval has been given to move ahead with this partnership.
DISCUSSION
Attached please find the draft request for proposal (RFP) and associated attachments
for local voice service, long distance services and internet services. The RFP was
compiled by The City of Dubuque, in partnership with Dubuque County working with
LINX Systems This work is part of the scope of services for the Telecommunications
Carrier Assessment Study LINX was hired to complete for both the City and County.
Key decision criteria for the RFP proposed are:
Services Value- Evaluation of Cost, service level agreements, type of service and
comparison to other like services.
Carriers Experience and Vision: Evaluation of the vendor's services and company's
history in providing new services and solutions.
Support /Service Capabilities: Serviceability of services provided, and technical
support for the Communications services provided. The ability to provide regional
support for services provided.
Scalability: Modular, cost - effective solutions in both IP services and voice services
over the next five years.
Leading Edge Technology: The ability to incorporate future requirements and
technological advances.
Financial and Corporate Viability: The ability to show a sound financial standing and
an ability to show cash reserves and other financial contingencies.
Cost savings over current that best matches needs and features of the City and
County while maintaining superior experience, services and support.

Responses must be submitted in sealed packages marked "Telecommunications

Dubuque County and City of Dubuque." Proposed costs for the pilot should be
included in a separate envelope titled "Costs for Telecommunications

1.6. Proposal Response Format
This RFP has been provided in soft copy for the proposing vendor's
convenience. Responses to Dubuque County's City of Dubuque and RFP
must be in the same order and format as the original request. All responses
should be stated in the body of the document following the specific questions.
Please place any additional reference information in the Appendix.
Note: Any modifications to the questions in this RFP by the proposing vendor
will result in immediate rejection of that proposal.
1.7. Site Information
1..7.1. Community Profile
Dubuque County serves an area of 608 square miles with a population of
93,653 (2010). The County's fiscal year begins on July 1 and ends on
June 30. Dubuque County is a multi- faceted organization which serves
the public through the offices of elected county officials; and provides
health care through the county Health department, which has a division for
environmental health and another for home health care; through
Sunnycrest Manor, a nursing home with separate units for geriatric care
and intermediate care for the mentally retarded; through an extensive
system of parks, a county library, through services to indigent Veterans

and other indigent persons; through an extensive array of services to the
mentally ill, mentally retarded and developmentally disabled; and through
its Secondary Road and Zoning departments.
Dubuque County is organized into 32 departments. The accounting and
financial reporting functions of the County are centralized.
Dubuque, Iowa is a progressive community in the upper Midwest.
Located along the Mississippi River at the intersection of Iowa, Illinois, and
Wisconsin, Dubuque has a population of 58,000 and serves as a
metropolitan service area for seven surrounding counties. The City is
approximately 30 square miles in area. The City's Fiscal Year 2014
recommended budget including capital expenditures is approximately
$150 million and the City provides a full range of services.
The community has a stable and diversified manufacturing base and a
growing service sector. Dubuque is the major retail, medical, educational
and employment center for the Tri -State area. Tourism continues to be a
major economic force in the community. City government continues to
work in collaboration with the private sector to promote economic
development.
This RFP is being issued jointly with options for vendors to propose
individual proposals for each entity as well as joint proposal(s) that may
address any economies of scale that may exist in partnering on such a
project.

1.9.Voice and Data Line Project Requirements
City and County of Dubuque seek a communications solution that will replace or
re- contract the current services. Your proposed services will seamlessly serve
the current locations, satellite locations, and any future projected growth.
The goal of this RFP is to provide services for City of Dubuque and Dubuque
County communications systems that will allow the City and County to control
costs while providing better service reliability. Further, both organizations are
interested in services that can be easily managed by the internal staff.

The overall scope of these services is to build a telecommunications system to
encompass all offices. All employees need access to the same local, long
distance, wide area network, and Internet.
The RFP needs to accommodate the following services. We prefer to
standardize on a single service provider but Local, Long Distance, data point to
point circuits, Internet, and DSL may be contracted individually based on bid and
package pricing.

1.9.2 Local Service Access to Emergency Services (911/E911)
Access to Emergency Services, Basic 911 (911) and Enhanced 911 (E911)
Emergency Service provides the capability to route end -users calls to an emergency
service bureau or the Public Safety Answering Point (PSAP), by dialing the 3 -digit
universal emergency telephone number 911. Proposing vendor must provide access
to emergency services either as a Reseller, Unbundled Network Element — Platform
(UNE -P), or Facilities -Based Competitive Local Exchange Carrier (CLEC). Access
to this service consists of the various components of a 911/E911 system, which
consists of 911/E911 trunking, 911/E911 routing, and the E911 database and

with Analog Centrex Lines. All Centrex lines approximately 350 lines will be moved
to other forms of analog lines 1 FB, Pots etc. Please provide information if the

phones services and service support at the Dubuque County Jail
• 911 Lines

9. IMPLEMENTATION PLAN
9.1.A brief description of the major steps in the implementation process.

9.4.Implementation Support
9.4.1. Identify the individual in your organization who will act as a focal point

implementation team, including titles and functional roles, and any

9.4.3. Describe your escalation procedure for addressing problems during
implementation.
10.TERMS AND CONDITIONS
10.1. Addenda
Addenda are any graphic or written instruments issued by the City of Dubuque and
Dubuque County prior to the date for receipt of proposals which modify or interpret
this document by additions, deletions, clarifications, or corrections. Addenda will be
mailed or faxed to all who are known to have received documents. No addenda will
be issued later than 72 hours prior to the time and date for receipt of proposals
except an addendum postponing or withdrawing the request for proposal.
10.2. Exceptions
Exceptions to any part of the requirements stated in this request must be clearly
identified as exceptions.
10.3. Confidentiality of Information
Any submitted information considered trade secret or confidential to the Proposer, or
rendered via a non - disclosure agreement with the City must be so labeled and
enclosed separately.
The Proposer, the City of Dubuque and Dubuque County and their agents will hold
the submitted proposal and any related materials in confidence if so requested by
the Proposer throughout the evaluation process. Any information therein will not be
made available to any other party unless precluded by Iowa Code. No debriefings or
scoring sheets will be released before final recommendation.
However, after the award, all contents of the selected proposal will be considered
public information.
All proposal material supplied, including supporting material and information
disclosed during the bid process, will become the property of the City of Dubuque
and Dubuque County and will be retained for internal use. The City of Dubuque and
Dubuque County reserve the right to retain all proposals submitted and to use any
ideas in a proposal regardless of whether that proposal is selected. Submission of a

proposal indicates acceptance by the Proposer of the conditions contained in this
request for proposals, unless clearly and specifically noted in the proposal submitted
and confirmed in the contract between the City of Dubuque or Dubuque County and
the vendor selected.
10.4. Incurring Costs
The City and County of Dubuque are not liable for any costs incurred with replying to
this RFP.
10.5. Caution to Proposers
The City of Dubuque and Dubuque County are not responsible for locating or
securing any information which is not identified in the RFP and reasonably available
to the City of Dubuque and Dubuque County. Vendors are encouraged to ask
questions to clarify items in this RFP that may need clarification.
10.6. Proposal Withdrawals
All requests to withdraw or resubmit a proposal must be made in writing to the City
of Dubuque and Dubuque County any time prior to the time of the proposal opening.
No Proposer may withdraw a proposal within 60 days after the actual date of the
opening thereof.
10.7. Evaluation Criteria
This RFP is not meant to favor any vendor or manufacturer. Instead, it is designed
to meet the needs of the City of Dubuque and Dubuque County. The City of
Dubuque and Dubuque County will weigh the proposals based on the Proposer's
references, qualifications and support as well as technical merit and cost. Award will
not be made on price alone.
In awarding the Contract, the City may take into consideration the Proposer's skill,
facilities, capacity, experience, responsibility, previous work record, financial
standing, the necessity of prompt and efficient completion of work described in the
proposal documents, or other factors the City of Dubuque and Dubuque County
considers relevant. Inability of Proposer to meet these conditions may be cause for
rejection of the proposal.
10.8. Proposal Acceptance
The City of Dubuque and Dubuque County reserve the right to accept or reject any
or all proposals and waive formalities or irregularities in the process. A proposal
once submitted shall be deemed final and binding on the Proposer, and shall
constitute an option with the City of Dubuque and Dubuque County to enter into a
contract upon the terms set forth in the proposal. All proposal prices must be valid
for 60 days from proposal due date.

10.9. Contract Negotiations
The City of Dubuque and Dubuque County reserve the right to negotiate a contract
after the successful Proposer is selected. Selection will be based only on the
proposal and subsequent interviews, if any; therefore, proposals must be complete.
10.10. Contract Documents
The Proposer's response to this RFP, response to questions and written addenda
will become part of the contractual documents upon signing of contract documents.
The order of precedence shall be signed contract, response to follow -up questions,
response to addenda and response to the RFP. The most recently dated response
to an item will supersede other items referencing the same topic. Should the vendor
show failure to perform as outlined in the defined contract documents, and failure to
rectify observed failures, termination of contracts will occur.
10.11. Damage Liability - The successful vendor is liable and responsible for
any damage to the premises (e.g., floor, walls, etc.) caused by vendor
personnel or equipment during installation and is responsible for the removal
of all project - related debris.
10.12. Permits - The vendor shall obtain and pay for any permits and
licenses required for the performance of the work, post all notices required by
law, and comply with all laws, ordinances and regulations bearing on the
conduct of the work, as specified herein. On any work which requires an
inspection certificate issued by local authorities, National Board of Fire
Underwriters, or any other governing body, such inspection certificate(s) shall
be obtained by and paid for by the vendor. The chosen vendor shall procure
all required certificates of acceptance or of completions issued by the state,
municipal or other authorities and must deliver these to City of Dubuque and
Dubuque County.
10.13. Vendor Responsibility - Unless otherwise stipulated, vendor shall
provide, and pay for, all materials, labor, tools, equipment, transportation,
and other facilities necessary for the performance and completion of the
work. Vendor shall verify conditions at the building, particularly door openings
and passages. Any pieces too bulky for existing facilities shall be hoisted and
otherwise handled with apparatus as required.
10.14. Terms and Conditions - The vendor shall include a copy of standard
terms and conditions as part of the system proposal. Include in Appendix D.
10.15. RFP Responses - All materials submitted by the vendor in response to
this RFP become the sole property of City of Dubuque and Dubuque County
upon receipt of the proposal. The material contained in these responses will

be appended to the final contract, further defining the contractual
responsibilities of the vendor.
