FEMA Buy -Out Program

Planning Services Manager Laura Carstens recommends City Council approval of an
amendment to the Cardno ATC Asbestos Survey and Monitoring contract for properties
at 1711 Old Mill Road and 1654 Manson Road.
The original contract provided for 35 samples divided between the two properties, with
the proviso that if additional samples were required, the City would pay for the actual
number of samples taken. Two additional samples were taken for a total cost of $36.
The new contract amount is $1,511. All other terms and conditions of the contract
remain unchanged.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

INTRODUCTION
This memorandum transmits an amendment to the Asbestos Survey and Monitoring of
Abatement Contract awarded to Cardno ATC. The contract was for the flood- damaged
properties at 1711 Old Mill Road and 1654 Manson Road (garage), and was awarded at
the March 18, 2013 City Council meeting. This contract is part of the Federal
Emergency Management Agency (FEMA) 404 Buy -Out Program. The FEMA 404
Program provides funds for the City to demolish flood- damaged properties.
DISCUSSION
The original contract provided for 35 samples divided between the two properties, with
the proviso that if additional samples were required, the City will pay for the actual
number of samples taken. In fact, two additional samples were taken at $18 each for a
total of $36. The new contract amount is $1,511. All other terms and conditions of the
Asbestos Survey and Monitoring Contract remain unchanged.
RECOMMENDATION
Planning staff recommends approval of the amendment to the Cardno ATC Asbestos
Survey and Monitoring contract and authorizing payment for the new contract amount.
REQUESTED ACTION
The requested action is for the City Council to approve the amendment to the Cardno
ATC Asbestos Survey and Monitoring contract for properties at 1711 Old Mill Road and
1654 Manson Road, and to authorize payment for the new contract amount.

PA CONTRACT AMENDMENT
This Amendment, mutually agreed to by City of Dubuque, Iowa, hereinafter referred to as "Applicant ",
and Cardno ATC , Contractor, and hereinafter so referred, hereby changes and amends a certain
Asbestos Survey & Monitoring Contract executed by Applicant and Contractor on February 27 , 20 13
WHEREAS, the aforementioned Contract calls for an adjustment of the Contract amount as result of
adjusting the 30 sanrples called for in the RFB to the actual number of samples taken, this Contract is
hereby amended as follows:
WHEREAS, the Contract in to the total amount of $ 1,475.00 , and
WHEREAS, the 30 samples bid for 1711 Old Mill Road and 5 samples for 1654 Manson Road was at a
price of $ 18.00 each and totaled $ 630.00 and
WHEREAS, the actual total number of samples taken totaled 37 , at $ 18.00 per sample,
THEREFORE, the Asbestos Survey & Monitoring Contract is amended as follows:
