SUBJECT: Tri-State Community Health Center FY 2007 Funding Request
During FY 2007 budget, the City Council approved funding in the amount of $250,000
towards the Tri-State Community Health Center (TSCHC) project. This project provides
capital and equipment funds for a TSCHC in Dubuque to be located at the former
Casket Company building.
The TSCHC would provide comprehensive primary health care and dental services,
regardless of ability to pay, creating a medical home for many low income people who
presently use local hospital emergency rooms or sporadic doctors' offices for their
source of primary care. Specific services that the TSCHC would provide include:
primary health care services by physicians and allied health professionals (Le., nurse
practitioner), oral health services provided by a dentist and dental hygienist, diagnostic
laboratory and x-ray services, preventive health services, patient case management,
pharmacy services, preventive and restorative dental services and translation services.
Support for this project is consistent with the City of Dubuque Comprehensive Plan,
Neighborhood Reinvestment Strategy and the Washington Neighborhood Reinvestment
Plan. This also helps achieve the Fiscal Year 2007 City Council Goals adopted in
September 2005, specifically a Diverse, Strong Economy with infrastructure and
amenities that support economic development and growth, including Quality of Life;
Planned and Managed Growth with safe, healthy neighborhoods; and Partnering for a
Better Dubuque by partnering for health care and wellness.
The Community Health Center was also identified by the community as one of the 10
projects to be completed by 2010 through the Envision 2010 process conducted by the
Community Foundation of Greater Dubuque and the Dubuque Area Chamber of
Commerce.
Most importantly, this proposal is consistent with the Fiscal Year 2007 City Council
Priorities and Major Projects adopted in September 2005, including Neighborhood
Crime Reduction Strategy" and "Casket Company Completion and Tot Lot."
The City of Dubuque will have no operational responsibility for the Community Health
Center and this is only a one-time Capital grant to help get the facility started. The City
is also assisting this project with economic incentives for the Washington Court
Apartments Project, the new neighborhood park, a Washington Neighborhood Resource
Center, and the creation of parking to serve patrons of the Community Health Center
and the Washington Neighborhood Resource Center.
I respectfully recommend approval of the attached resolution authorizing distribution of
the one time $250,000 grant.

AUTHORIZING THE DISTRIBUTION OF $250,000 TO THE TRI-STATE
COMMUNITY HEALTH CENTER IN DUBUQUE FOR CAPITAL AND
EQUIPMENT FUNDING
WHEREAS, the City Council, in its fiscal year 2007 budget, budgeted
$250,000 to be used by the City of Dubuque (City) to provide a one time
grant to The Tri-State Community Health Center (TSCHC) for capital and
equipment costs; and
WHEREAS, TSCHC will provide comprehensive primary health care
and dental services, regardless of ability to pay, creating a medical home
for many low income people who presently use local hospital emergency
rooms or doctors' offices for their source of primary care; and
WHEREAS, the City's support for the TSCHC project is consistent
with the City of Dubuque Comprehensive Plan, Neighborhood
Reinvestment Strategy and the Washington Neighborhood Reinvestment
Plan and is also consistent with the Fiscal Year 2007 City Council Priorities
and Major Projects adopted in September 2005, including "Neighborhood

WHEREAS, TSCHC has now requested distribution from City in the
amount of $250,000.00 for the one time grant to help with start up costs.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF
THE CITY OF DUBUQUE, IOWA:
The City Finance Director is authorized and directed to distribute
$250,000.00 to TSCHC for the capital and equipment funding in support of
the TSCHC project.
