Van Milligen City Manager of Dubuque 50 West 13th Street Dubuque, Iowa 52001 -4864 RE: Fourth Quarter Report - FY 99 Dear Mr. Van Milligen: As per our Purchase of Services Agreement with the City of Dubuque, I am making this Fourth Quarter Report detailing our activities in working with youth from Dubuque who have been referred to our High Risk Youth programs. During the fourth quarter, the following numbers of youth in the City of Dubuque were seen in the following programs: Crisis Contacts Individual/Family Counseling Therapy /Support Groups Educational Presentations 4th Quarter Year -To -Date 16 youth 161 youth 152 youth 100 youth 78 youth 677 youth 599 youth 700 youth P1 118 West Main P.O Box 31 Manchester, Inwa 52057 -1534 (319) 927 -5112 Fax (319) 927 -3340 Additionally, during Fiscal Year 1999, 113 youth participated in activities at the CHOICES Program, the drop -in center for youth. The CHOICES Program was not refunded by the Iowa Department of Public Health for Fiscal Year 2000. The drop -in center ceased program operations on June 30, 1999. SASC anticipates no other changes in its youth programs during Fiscal Year 2000. Our Board of Directors and staff greatly appreciate the support we receive from the City of Dubuque. Thank you.
