In the past, the City of Dubuque has very graciously written letters of support/commitment
for the Tri-State Community Health Center's efforts to obtain grant funding to establish a
Federally Qualified Community Health Center (FQHC) in Dubuque. We are submitting
another application, in order to secure the ongoing viability of Crescent Community Health
Center (CCHC). Your continued support is vital to the grant application.
Please include in your letter any statistics or increases your organization has experienced
serving persons with low income, Hispanics, minority groups, uninsured/underinsured
and/or immigrants. Also, please be specific in how you will partner/collaborate with CCHC.
Also notice the name change due to the generosity of the Schmid family. For your
information the Center has begun to see dental and medical clients. Thank you for your
support.

The City of Dubuque supports the Crescent Community Health Center's (CCHC) application for a
federally qualified health center. Although the Crescent Community Health Center is not a City facility or
department, the City of Dubuque has been a proud partner in its creation. In September 2004, the City
Council identified "regional healthcare center promotion" as an action item in the 2004-2010 Strategic
Plan. "Community Health Center Project and Funding" was an element of the City Council's 2005-2007
Policy Agenda and it was listed as a top priority in the 2006-2007 Management Agenda. The community
health center's location is also a key component of the Washington Neighborhood Revitalization
Strategy.
Specifically, the City's support of the community health center project includes the involvement of
Dubuque Public Health Specialist Mary Rose Corrigan, the contribution of $250,000 for capital build-out
and an additional $647,000 in funds and staff oversight for the parking lot development and construction,
and providing office space for the health center staff while the facility was completed. The City also
acquired the necessary property and created a new neighborhood park across the street at a cost of
$320,000 as part of the neighborhood revitalization and to compliment the community health center.
Over the past several years, many organizations and planning efforts throughout the community have
identified primary health care, particularly for those who are uninsured or under-insured, as a critical
need throughout the Dubuque area. In addition to the efforts of Mercy Medical Center's Community
Assessment of Human Needs, the Loras/Clarke College Community Outreach Center Grant Committee,
and the Visiting Nurse Association's Strategic Planning Effort, the City's Comprehensive Plan specifically
addresses access to adequate health care services. Promoting access to adequate health services for
all members of the community and addressing special health needs of pregnant women, children and the
elderly are goals in the City's annually updated Comprehensive Plan.
I am impressed how such a wide variety of individuals and organizations from throughout the Dubuque
community have come together to plan for the CCHC. With the assistance of HRSAlBPHC this center
will remain a viable health care safety net for our community.
Dubuque is the only metropolitan area in Iowa without a community health center. The community has
come together to create a health center and, not only is City of Dubuque staff involved in this process,
we fully support the effort. I know you will look favorably upon Dubuque's application and feel confident
that you have the support of the entire community.
