SUBJECT: Acceptance of the Renewal Rates for the Voluntary Dental Plan
Personnel Manager Randy Peck is recommending City Council approval of the renewal
rates for the Voluntary Dental Plan, which is paid in full by the employees. The single
premium rose from $30.59 per month to $32.12 per month, and the family plan rose
from $91.72 per month to $96.31 per month.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

SUBJECT: Acceptance of the Renewal Rates for the Voluntary Dental Plan
The premium for the dental plan, which is paid by the employee, will increase from
$30.59 per month to $32.12 per month for the single plan and from $91.72 per month to
$96.31 per month for the family plan. These rates would be in effect from January 1,
2007 through December 31, 2007. Employees who enroll in the dental plan will have
the payments deducted from their first two paychecks each month. The premiums will
be divided in two and they have to be an even number. Consequently, I recommend
that the amount employees will pay for the family premium be reduced from $96.31 per
month to $96.30 per month effective January 1, 2007 through December 31,2007. The
City will be billed at the rate of $96.31 per month for employees who enroll in the family
dental plan. The requested action is for the City Council to approve the renewal rates
and authorize you to sign the attached financial exhibit.
