The application asks that IBM experts assist in mining existing data and exploring new data sources from a variety of health and social sectors and determine technology needs for gathering and analyzing the data. There is no financial requirement for this application. If chosen, Dubuque would have the option of reviewing any recommendations made by the consultants and implement only those the City of Dubuque feels are appropriate and fruitful. BACKGROUND The Smarter Cities Challenge contributes the skills and expertise of IBM's top talent to address critical challenges facing cities around the world. They do this by putting teams on the ground for three weeks to work closely with city leaders and deliver recommendations on how to make the city smarter and more effective. Over the past three years, 100 cities have been selected to receive these technical assistance grants. The Smarter Cities Challenge is IBM's largest philanthropic initiative, with contributions valued at over $50 million to date. DISCUSSION To plan for a community -wide health and wellness initiative and advance the Smarter Health Project, local data must be collected and analyzed from a variety of sources, including non - health specific data sets. Crescent Community Health Center offers high - quality and affordable care to anyone needing a medical and /or dental home, including individuals and families who are uninsured, underinsured and those with various types of insurance. • Mercy Medical Center - Dubuque is a general medical and surgical hospital, with 272 beds. • Unity Point - Finley Hospital is a general medical and surgical hospital with 119 beds. It is one of two hospitals in Dubuque. • Medical Associates Clinic is a multi - specialty medical group practice that provides primary and specialty health care services to residents of the tri -state area. • Dubuque Internal Medicine has over 35 physicians and over 300 employees. Dubuque Internal Medicine is the largest internal medicine group practice in Iowa. • Tri -State Independent Physicians Association is a multi - specialty group practice representing a full array of primary care and surgical specialties. What systemic factors prevent the city and community from addressing this issue without a Smarter Cities Challenge grant (for example: insufficient budget, organizational culture, regulation, legislation, etc.)? • Systemic Factors Prevent the City and Community from Addressing this Issue without a Smarter Cities Challenge Grant Previous community health initiatives in Dubuque have had limited impact due to the absence of insights from City data on multi - sectoral dynamics and leverage points for wellness and health in communities. Limited impact has also been related to external funding mechanisms with a vertical, program specific focus and lack of robust evidence on the effectiveness (cost and /or clinical) or the persistence of benefit after cessation of time limited programs. Numerous health and wellness organizations and professionals have designed many health and wellness initiatives. However, sustained health and cost improvements have not been demonstrated within the community, nor has any in -depth evaluation of the efforts occurred. Anticipated Outcomes A. What would be successful short and long -term outcomes of a Smarter Cities Challenge engagement for the city? • Successful Short and Long -Term Outcomes The key deliverable in the Challenge Grant's work is an assessment and a framework with recommendations and estimated costs on: the data (which are relevant, accessible, usable, ingestible), the requisite data technologies, the data base, storage and security needs and technologies, the analytic methods and visualization technologies to consider, and how to use these to identify high impact problems and potential interventions. Data would need to be inventoried and then evaluated, assessed, and classified. We would also need to determine technology needs and how to use these to define the specific problem for which the community health initiative would be directed, along with a budget and finance plan for how to move forward. Any community -wide health initiative must have a solid business plan. The long -term goal for the outcomes developed through the Smarter Cities Challenge would be to develop analytics of health, social, and environmental data as it relates to systems, and Sustainable Dubuque and Smarter Sustainable Dubuque efforts as they are occurring. All of the City's efforts around Sustainable Dubuque and Smarter Sustainable Dubuque have utilized citizen engagement data and technology. We would utilize previous Smarter City engagement strategies and build upon those successful activities to create even stronger engagement targeted specifically to the health care and public health sectors in the plan and moving forward with the initiative. B. After the engagement, what specific actions would be taken to implement recommendations? What efforts would be taken to share best practices with other cities and regions? • First and foremost, the Dubuque City Council would incorporate the results of the Smarter City Challenge into the City Council Goals and Priorities under the topic of the Community -Wide Health and Wellness Initiative or Smarter Health. Secondly, this proposal will tell us how the data and metrics will be obtained which will be the fundamental piece of work that's necessary for a larger proposal for community -wide health and wellness. Before we can plan for this larger endeavor, we need to collect critically and analyze local data in order to assure we are addressing the real needs. This, along with the Smarter Cities Challenge revealing what technology is needed, will become a funding priority for the City. The City of Dubuque has a long history of creating community partnerships and utilizing a variety of funding sources including local, state, and federal dollars with private sector and philanthropic sources for large- scale community projects. Our history with Senator Tom Harkin, support of the White House, and other federal and state agencies and private organizations reveal that Dubuque can secure funding sources and utilize them to their fullest benefit. Capacity for a Driving Change A. Please describe the city's accomplishments in encouraging innovation and driving change, that demonstrate the city's and city leader's capacity for acting on recommendations delivered by a SCC grant. • Capacity for a Driving Change The City of Dubuque has partnered with IBM Research on numerous pilot projects since 2009, all of which fall under the "Smarter Sustainable Dubuque" initiative. Through this partnership, the City and IBM have worked to implement technologies that provide residents with information on services such as water and electricity usage, transportation, physical activity, and solid waste disposal. The goal of these pilots it to identify how citizens' behavior responds simply to having access to this valuable information. Due to this continued partnership, the City has developed and demonstrated the capacity to implement recommendations delivered by a Smarter Cities Challenge grant. Data A. What role can open data and citizen engagement play in addressing the proposed topic area? • Data from health care providers in the community is currently limited to hospital- based, reportable diseases, behavioral risk factor surveillance survey data and other state - gathered information. This points only to a small snap shot of potential health needs in the community. Data from a variety of health and social and other multi - sectorial sources would create the ability to assess specific health and wellness related needs and also points to the specific areas (health, housing, diet, neighborhood, disability, etc.) that should be addressed in order to effectively meet the need. Because our current identified needs are based on broad, non - specific and strictly health data sets, we are often putting our resources and efforts into activities which may or may not be the cause. With data able to drive at the real needs, community engagement strategies can also be targeted at specific populations using appropriate strategies. B.
