This written report constitutes your claim against the City of Dubuque, Iowa. You
should complete this form in full and attach any additional information that
supports your claim.
The claim must be filed with the City Clerk at City Hall, 50 West 13th St.,
Dubuque, IA 52001. It will then be referred to the appropriate department for
investigation and to the City Attorney's Office. Once that investigation is
completed, a report and recommendation will be submitted to the City Council.
You will be provided with a copy of that report and recommendation.
The final decision on all claims is made by the City Council. No employee of the
City of Dubuque has the authority to make any representation to you as to
whether your claim will or will not be paid.

Dear Ms. Bramwell:
Our client, Mr. Schumacher, desires to file a claim against the City of Dubuque, Iowa for the
damage that was caused at his home by Matt Kufman who was hired to do the drywall work. Mr. Kufman
pulled the stove out from the wall in Mr. Schumacher's home and broke the leg off the stove and damaged
the vinyl flooring. Mr. Schumacher cannot use the stove and there is damage to his vinyl floor. Enclosed
please find an estimate to repair the vinyl flooring in his home as well as the receipt for the purchase of a
new stove. If the City is denying any liability with regard to this claim, please let me know as soon as
possible. I would like to get Matt Kufman's information as well so that I may send a copy of this claim to
him. If you could forward that information on to me, I would greatly appreciate it.
