SUBJECT: Floodwall Improvement Projects -Change Orders #4 and #5
INTRODUCTION
The purposes of this memorandum are to provide information and request your action
regarding the attached change orders for the Floodwall Improvement Projects currently
underway.
BACKGROUND
The Floodwall Improvement Projects, providing auxiliary power to the floodwall's Maus
Lake and Hawthorne Street pumping stations, along with numerous infrastructure
enhancements at the Bee Branch Pumping Station, are currently underway. The total
budget appropriation for the projects is $555,766.00 - $446,420.80 for the construction
contract, $66,400.00 for engineering fees, and, $42,945.20 for contingencies.
DISCUSSION
A memorandum from Street and Sewer Maintenance Supervisor John Klostermann
regarding proposed change orders #4 and #5 is attached. Also attached are the noted
change orders in triplicate. The work associated with the change orders is considered
essential for the improvements at the Bee Branch and Hawthorne pumping facilities.
The total cost of the proposed change orders is $4,920.60. When added to the three
previously-approved change orders, the total change order cost would equal
$19,310.01. This would leave a contingency fund balance of $23,635.19. The
additional cost of the five change orders would equal 4% of the original construction
contract for the projects.
ACTION STEP
Your endorsement of the attached change orders is recommended and requested.

Introduction
The purpose of this memorandum is to request your approval of change orders #
4 & 5 for the Floodwall Improvement Projects at Hawthorne Street, Bee Branch
and Maus Lake Pumping Stations.
Discussion
Change order # 4 is for various types of necessary construction related changes
associated with the improvements being made at the Bee Branch Floodwall
Pumping Station. These changes include pouring a return wall from the end of
the retaining wall back to the existing building foundation and to pour a concrete
pad between the sidewalk and existing building.
Change order # 5 is for the Hawthorne Street project. The existing 5KVA
transformer located in the building was replaced with a 10KVA transformer to
serve the new equipment associated with the generator. I have reviewed the
change orders as submitted with Charles Cate of IIW and we both feel that the
change orders are fair and just.
Action Required
I am requesting that you review change orders #4 and #5 and recommend
payment.

2) Pave area between the stair and existing building south corner
from sidewalk curb to existing building stoop slab after
installation of fence post.

Justification for Change:
$1, 815.60
1) The elevation difference of the grade behind the retaining wall and the grade to the
south of the retaining wall termination point is too great.
2) Slope is too great to maintain clean stone.
Change in Contract Price:
The amount of the contract will be Increased by this change order by the sum of
One Thousand Eight Hundred Fifteen and 60/100 Dollars $1,815.60

Improvement Bond and insurance all work covered by this change order.
All work performed under this change order shall be performed in accordance with. the
requirements of this change order, the plans, the specifications, and the contract documents.

De-energize and remove existing 5KVA transformer from
MCC. Furnish and install new 10KVA transformer along with
fused disconnect and associated fuses. Lump Sum $3,105.00
Justification for Change:
Existing transformer did not carry a high enough rating to serve the new equipment. Existing
transformer replaced with a new higher capacity transformer.
Change in Contract Price:
The amount of the contract will be Increased by this. change order by the sum of
