A resolution providing for the approval is attached for the Mayor's signature. BACKGROUND The Community Partnership (cp2) program was implemented on August 5, 1991, to provide financial support to new and innovative private and non-profit efforts to create a more livable environment for community residents. Funds are allocated for the cp2 program on a yearly basis through the CDBG Annual Plan process. A total of $1,641,394 has been awarded since the program began. The program has leveraged over $6 million in community service and accessibility projects. Currently the Community Partnership (cp2) program was allocated $50,000 for FY 2008, with all of the funds initially proposed for public service activities. However, an amendment to the FY 08 Annual Plan due to a reduction in our entitlement allocation is currently being considered by the Community Development Advisory Commission which would reduce cp2 funding to $49,348. This amount was used for the cp2 allocation recommendation. The Housing and Community Development Department announced the availability of cp2 funds on February 12th and held an informational meeting in the auditorium of the Carnegie Stout Public Library on February 21st. Applications were accepted through March 28th. Six applications were received requesting a total of $79,856. DISCUSSION A Review Committee was established consisting of: Pat Prevenas, Recreation Division Manager; Janet Walker, Assisted Housing Supervisor; Dave Johnson, Assistant Planner; Jerelyn O'Connor, Neighborhood Development Specialist; Aggie Tauke, Community Development Specialist; Community Development Advisory Commissioners Janice Craddieth and Hillary Stubben. The cp2 Review Committee evaluated each of the proposals and ranked them in accordance with the guidelines adopted by the City Council. The Committee's report and a brief description of the proposals and ranking of each are attached. The Community Development Advisory Commission (CDAC) met on Wednesday May 9th to consider the following cp2 applications. At that time, the Commission reviewed the review committee's recommendations and proposed funding the projects as recommended. The cp2 program requires 25% match for the initial year of funding. Based on the desire to fund as many projects as possible, the Commission is proposing to fund the top four ranking applications. Since the $49,348 of cp2 funding is less than the total requested for these projects, application for Audubon School and Hillcrest Family Services will be funded at eighty-three percent of their request. This percentage is based on the total available divided by the total requested by the four applications. The Crescent Community Health Center (CCHC) application for expansion of the dental facilities will be funded at less that eighty-three percent in order to fully fund the application for the Dubuque Visiting Nurse Association (VNA), which would provide case management for the CCHC. Mary Rose Corrigan, Interim Executive Director of CCHC, has concurred with this funding recommendation. The applications for the Dubuque County Historical Society and Consumer Credit Counseling are not recommended for funding. ACTION STEP The action step for the City Council is to adopt the attached resolution approving the award of $49,348 of cp2 funds to the aforementioned organizations and authorizing the City Manager to prepare and execute the necessary funding agreements. Following is a brief summary of each of the cp2 applications received and evaluated by the Review Committee. Additional information relative to the various proposals and the individual evaluations are available upon request. The Committee reviewed and ranked the applications based on established criteria. Based on the desire to fund as many projects as possible, the Committee proposed to fund the top four ranking applications. Since the $49,348 of cp2 funding is less than the total requested for these projects, the Committee has proposed to fund each application at eighty-three percent of their request. CRESCENT COMMUNITY HEALTH CENTER Project Description: Dental Office Expansion This project provides for expansion of the dental facilities at the existing Crescent Community Health Center (CCHC) at 1789 Elm Street. Total Project Cost: CP2 Request: National Objective: CDBG Eligible Activity: Consolidated Plan: Performance Outcome: Evaluation Points: Recommendation: $139,418 $25,418 Low/moderate clientele Public Facilities and Improvements Theme 2 Goal 1 Objective 2 Accessibility 96 $19,216 Comments: The Committee found the project would provide for 3 additional operatories to serve the dental needs of underserved tow-income individuals and families. Since the application for the Dubuque Visiting Nurse Association (VNA) would provide case management for the CCHC, the Committee recommended that if the Health Center concurred, funding for the CCHC would be reduced to fully fund the VNA application. Interim Executive Director Mary Rose Corrigan has concurred with this funding recommendation. AUDUBON SCHOOL Project Description: Plugged in to Reading: Home Connected to School This project provides for ahome-school literacy program for grades 3 to 5 with take-home reading kits containing a book/cd and cd player to improve reading fluency, provide encyclopedias in the classrooms which can be taken home by students, and also give free paperback books in conjunction with the family literacy nights at Audubon School. The Committee proposed to fund the application at eighty-three percent of their initial request. HILLCREST FAMILY SERVICES Project Description: Project in Transition from Homelessness (PATH) This project would expand the PATH program (Projects in Transition from Homelessness) by increasing apart-time program counselor to full time and providing utility and rent deposits for homeless mental ill persons. Total Project Cost: CP2 Request: National Objective: CDBG Eligible Activity: Consolidated Plan: Performance Outcome: Evaluation Points: Recommendation: $ 45, 305 $ 11,523 Low/moderate Housing/ Clientele Public Services/Security deposits Theme 1 Goal 4 Objective 1 Availability/Accessibility 90 9, 555 Comments: The Committee found the program would provide needed services to a disadvantaged population. The program would also facilitate access to permanent housing by assisting with rent and utility deposits. The Committee proposed to fund the application at eighty-three percent of their initial request. VISITING NURSE ASSOCIATION Project Description: VNA/CCHC Case Management Project The project would provide case management to the Crescent Community Health Center (CCHC) to address healthcare access issues, preventive care and assessing other client needs. Total Project Cost: CP2 Request: National Objective: CDBG Eligible Activity: Consolidated Plan: Performance Outcome: Evaluation Points: Recommendation: $ 14, 520 $ 10,890 Low/moderate income clientele Public Services- Health Services Theme 2 Goal 1 Objective 2 Avai labi I ity/Accessibi I ity 87 $10,890 Comments: The Committee found the project will provide needed services to low income persons who receive care at the CCHC. The Committee expressed concern for the need to fund the entire request to facilitate the program's success. Therefore, based on the VNA's cooperative relationship with CCHC, they recommended full funding if CCHC concurred with a funding reduction. CONSUMER CREDIT COUNSELING Project Description: Consumer Credit Counseling of Greater Dubuque Area This project provides for educating low-and moderate-income families on the financial aspects of buying a home with the NMake Your Move" program and with atenant/landlord course addressing rental issues, "Renting Today". Total Project Cost: CP2 Request: National Objective: CDBG Eligible Activity: Consolidated Plan: Performance Outcome: Evaluation Points: Recommendation: $ 9,600 $ 7,200 Low/moderate income clientele Housing Services Theme 1 Goal 1 Objective 2 Availability/Accessibility 60 0 Comments: The Committee expressed concern with the ability to obtain participants for the programs. They also noted that a similar program is provided for first time homebuyers by Iowa State University Dubuque County Extension office. No funding was recommended. Dubuque County Historical Society Project Description: Removing Challenges: ADA Doors The proposed project would install accessible powered doors in the National Mississippi River Museum & Aquarium. Total Project Cost: $ 17,523 CP2 Request: $ 13,142 National Objective: Low/moderate income clientele CDBG Eligible Activity: Not eligible Recommendation: 0 Comments: The project would provide push button switches to the existing accessible doors to the museum. Since the building is currently accessible, the request does not meet the CDBG requirements for removal of architectural barriers. No funding is recommended. Whereas, under the provisions of Title I of the Housing and Community Development Act of 1974, as amended, the City of Dubuque has received Community Development Block Grant {CDBG) funds for the development and maintenance~f a viable urban community through the provision of decent housing, a suitable living environment and expanded economic opportunities for its residents, particularly those persons of low and moderate income; and Whereas, the Community Partnership Program {cp2) was developed in the City of Dubuque in response to the national goals and objectives of the CDBG program; and Whereas, guidelines for the implementation of the Community Partnership Program (cp2) were approved and amended by the City Council. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DUBUQUE, IOWA: Section 1. That the City Council hereby awards Community Partnership Program .(cp2) monies to the following organizations.
