The City of Dubuque was awarded a Phoenix Housing Continuum of Care Grant in the
amount of $102,115.00 to provide permanent, supportive housing for disabled homeless
households. The City of Dubuque contracts with Project Concern for Sponsor-Based
Rental Assistance.
The program provided rental assistance for 29 dwelling units affecting 41 persons in the
amount of $95,568. The grant included $6,547.00 in administration fees of which
$3,855.00 was paid to Project Concern. Project Concern and other area providers
contributed supportive services in an amount of$315,197.24 during the operating year.
This one year grant expired June 30, 2014.
Housing and Community Development Director Alvin Nash recommends City Council
authorization for the Mayor to sign the close-out documents for submittal to HUD.
I concur with the recommendation and respectfully request Mayor and City Council
approval.

INTRODUCTION
This memorandum requests City Council authorize the Mayor to sign documents to
close out the FYE 2014 Continuum of Care Grant for the Phoenix Housing Program
(formerly Shelter Plus Care).
DISCUSSION
The City of Dubuque was awarded as a one year grant in Federal Fiscal Year 2012 in
the amount of$102,115.00 for Phoenix Housing Continuum of Care Grant to provide
permanent, supportive housing for disabled homeless households. Grant Number
IA0061 L7D011202 was for one year beginning July 1, 2013. The City of Dubuque
contracts with Project Concern for Sponsor-Based Rental Assistance (SRA). The grant
expired as of June 30, 2014; therefore, we are processing the grant for closeout.
The program provided rental assistance for 29 dwelling units affecting 41 persons in the
amount of $95,568. The grant included $6,547.00 in administration fees of which
$3,855.00 was paid to Project Concern. Project Concern and other area providers
contributed supportive services in an amount of$315,197.24 during the operating year.
A grant closeout certification must now be executed by the City to proceed with the
closeout. The City Council must approve execution of the certification to facilitate
submittal of closeout documentation to HUD.
ACTION REQUESTED
I request that the City Manager recommend City Council approval of the attached
Continuum of Care Grantee Closeout Certification authorizing grant IA0061 L7D011202
Phoenix Housing Project be closed. This will allow the City to submit necessary
documentation to HUD to close the one-year grant.

City of Dubuque was awarded a Fiscal Year 2012 Supportive Housing Program Grant in
the amount of $102,115.00 for the Phoenix Housing Program (Grant No IA0061L7D011202).
The grant award was for 1.0 year beginning July 1, 2013. As the grant has expired, we are
processing the grant for closeout.
To the extent that any funds paid with the Supportive Housing Program Grant have not
been audited, such funds are subject to coverage in the next single audit. The grantee may be
required to repay HUD any disallowed costs based on the results of the audit.
Enclosed are three copies of the Continuum of Care Grantee Closeout Certification.
Please execute all three copies and return two of the certifications to this office. The remaining
copy is for your records. Also, please provide an inventory of any real property and equipment
purchased with grant funds.
