Van Milligen, City Manager SUBJECT: Fire Station #4 Driveway and Sidewalk Replacement Project DATE: December 28, 2015 City Engineer Gus Psihoyos is recommending acceptance of the construction contract for the Fire Station #4 Driveway and Sidewalk Replacement Project, as completed by Midwest Concrete Inc., in the final contract amount of $23,670.08, which was $7,037.98 over the contract award. The driveway was replaced at the same elevation as existed, however with the L512 Rosenbauer, 102' articulating platform truck now at this station had difficulty exiting without twisting the chassis. Therefore, the added construction costs were due to a portion of the driveway, walk and approach being replaced and lowered to alleviate any additional wear and stress on the equipment. BACKGROUND The project included the replacement and widening of the driveway, approach and defective public sidewalk along Atlantic, University and N. Grandview. DISCUSSION Midwest Concrete Inc. completed the replacements for a final contract amount of $23,670.08 and authorize final payment to the contractor. The driveway was replaced at same elevation as existed, however with the L512 Rosenbauer, 102' articulating platform truck now at this station, the equipment had difficulty exiting without twisting of the chassis. Jane Smith, EAII FAJANE SMITH\PROJECTS\Fire House#4 University at N Grandview 2 RESOLUTION NO. 2-16 ACCEPTING THE FIRE STATION #4 DRIVEWAY AND SIDEWALK REPLACEMENT PROJECT AND AUTHORIZING THE PAYMENT OF THE CONTRACT AMOUNT TO THE CONTRACTOR Whereas, the Public Improvement Contract for the Fire Station #4 Driveway and Sidewalk Replacement Project (the Project) has been completed and the City Engineer has examined the work and filed a certificate stating that the Project has been completed according to the terms of the Public Improvement Contract and that the City Engineer recommends that the Project be accepted; and NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DUBUQUE, IOWA: Section 1. The recommendation of the City Engineer is approved and the Project is hereby accepted. Section 2. The Finance Director is hereby directed to pay to the Contractor from the Driveway Replacements and City Catch Basin & Curbing appropriations for the contract amount of $23,670.08, Tess any retained percentage provided for therein as provided in Iowa Code chapter 573, and to pay such retainage only in accordance with the provisions of Iowa Code chapter 573.
