You should complete this form in full and attach any additional information that supports your claim. The Claim must be filed with the City Clerk at City Hall, 50 W. 13th St., Dubuque, IA 52001. It will then be referred by the City Council to the appropriate department for investigation. Once that investigation is completed, a report and recommendation will be submitted to the City Council. See attached letter: December 20, 2001 To: City of Dubuque Water Dept., From: Kenneth & Marla Meyer: Re: Water Damage from Meter Installation May 2, 2001 To Whom It May Concern: On May 2, 2001 per the city's request, we had our water meter relocated to the outside of our house for the convenience of the meter readers. The next day we found our living room carpet saturated with water coming from the utility room where the water meter previously was located. I placed an after-hours call to your office and a service person arrived to assess the problem. He discovered a "coupling" was not tightened completet from the previous days service work, which led to the leaking of water. I had him look at the water level on the living room floor and when he stepped on the carpet the water rose over the top of his work boot. He suggested we contact a carpet cleaning to remove the water. We contacted Kramer Carpet Cleaning that evening and they arrived the following moring - May 4, 2001. The carpet had to be pulled from the wall and the and the pad underneath was permanently damaged and was removed due to teh saturation. He treated the floor and carpet and provided blowers to dry the wet area. It took approximately 4 days for the carpet to completely dry. He then removed the blowers and reattached the carpet to the floor. Within a week of this situation, we again found our living room carpet saturated with water. We placed a second call to your office and a second service person came to fix the problem. He assessed that the "coupling" wa snow cracked - his guess was it was over-tightened. He suggested we again contact the carpeting cleaning service to remove the water, get an estimate to replace the carpet and padding, and submit the damage claim to your office. In addition to the carpet damage, we also had water damage to the wall. Enclosed with this letter are copies for the costs to repair and replace the damaged carpet adn surrounding areas - Kahle Carpets $1699.00 (to replace the carpet and pad with comparable carpet and pad), Joe Faley Construction $475.00 (to repair and repaint the wall due to the water damage) and also the bill from Kramer Carpet Cleaning $286.20 for the water clean up. We are requesting reimbursement for the amount of $2460.20 to cover the expenses. Thank you for your cooperation and attention to this matter.
