Background The Continuum of Care SNAP provides rental assistance to homeless persons with disabilities. The Phoenix Housing Project has been in operation in Dubuque since 2007. The project's goal is to provide permanent, supportive housing to homeless individuals/families with disabilities. Agreements in place clarify duties of participants, private housing providers, the City of Dubuque Public Housing Authority(PHA), the Sponsor Agency, Project Concern, and referring agencies. Project Concern is the primary contact with the client. The City of Dubuque serves as the grantee with administration, inspection and fiscal responsibilities. Discussion The one-year renewal application is due to the Iowa Balance of the CoC at IFA by June 24, 2016. The renewal application will assist with approximately 14 households in the amount of$78,691 for City of Dubuque FYE 2018. Project Concern provides case management to participants (clients) which includes: setting goals related to housing, increasing income and/or skills and setting personal goals in order to create an overall better quality of life. The goals are set by the client; the coordinator is there to assist clients with referrals, skill building and for overall support. The coordinator prepares a monthly supportive service report for each participant to keep track of services accessed by the client, including mental health services, substance abuse treatment and/or job training programs, as needed. Project Concern works with apartment owners to secure suitable housing located at scattered sites throughout the community and schedules appointments bi-monthly at the housing unit to assist clients with housing needs or concerns. The duration of the rental assistance is unlimited although the goal is to provide enough support, guidance and services so that the individual/family is able to live independently without fear of becoming homeless again. Included in the priorities of the City of Dubuque Consolidated Plan is to increase the housing options and related services for disabled persons and homeless individuals/families. Action Step The action requested of the City Council is to authorize an application to IFA and to HUD for the CoC SNAP grant for the one-year renewal application. Dubuque@Home is an initiative of the City of Dubuque Housing&Community Development Department 350 W.6`h St.,Dubuque,Iowa 52001 1 Phone: 563.559.4230 1 www.cityofdubuque.org/dubuqueathome RESOLUTION NO. 236-16 RESOLUTION AUTHORIZING THE MAYOR TO EXECUTE AN APPLICATION FOR THE CONTINUUM OF CARE GRANT AND AUTHORIZING THE DIRECTOR OF HOUSING AND COMMUNITY DEVELOPMENT AND THE CITY MANAGER TO APPROVE THE APPLICATION Whereas, the Iowa Finance Authority on behalf of the Iowa Balance of the Continuum of Care has published notice requesting Federal Fiscal Year 2016 Project Applications for Renewal Projects; and Whereas, the U.S. Department of Housing and Urban Development has published the availability of the Continuum of Care grant for the Collaborative Applicants including the Iowa Balance of the Continuum of Care; and Whereas, the U.S. Department of Housing and Urban Development will publish the availability of the Continuum of Care grant for the Project Applicants including the City of Dubuque Phoenix Housing Project; and Whereas, the City of Dubuque has administered the Continuum of Care grant since 2007; and Whereas, the City of Dubuque proposes to continue administering the Continuum of Care grant to assist homeless, disabled persons with permanent supportive housing; and NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF DUBUQUE IOWA: Section 1: That the Mayor is hereby authorized and directed to submit an application to IFA and HUD for the Continuum of Care program grant; and Section 2: That the Director of Housing and Community Development and the City Manager are hereby authorized to approve the application that is submitted to IFA and to HUD Section 3: That the City Manager is hereby authorized and directed to forward said application and resulting standard executed contract to the respective agencies in a timely fashion and as required by the Iowa Finance Authority and the U.
