Experience or I have experience sitting on a board that crosses jurisdictional
education which boundaries. I have been on the Board of Directors for
qualifies you for this Operation: New View Community Action Agency for about 12
board or commission: years. That board is made up of public and private actors from
Delaware, Dubuque, and Jackson County. My education is in
law which lends itself to long-term planning, thinking,
deliberative thinking, and problem solving, all of which I think fit
the commission's mission statement of addressing water
quality and flooding as well as working with others. In addition, !
water quality has been a concern of mine for some time and I
have educated myself on the topic by doing research including
reading public policy and scientific reports.
What contributions can I believe I can contribute by using the skills I mentioned above
you make or state (e.g.: long-term planning, etc.). My legal experience and j
reason for applying? training can also be helpful in developing policies to further the
commission's mission. I am applying because water quality is
something that I feel very strongly about and want to help serve
the area. I am also applying because I was trying to decide
between applying for the watershed authority or the resilient
community advisory commission. While researching both, I saw j
that three of the members of the resilient community advisory
commission should also be.members of other commissions, jJ
including the watershed authority and thought I would be a
good fit for both.

Experience or Education: I attended Ohio University in Athens, Ohio. I
education which majored in journalism, earning a Bachelors of Science from the
qualifies you for this E.W. Scripps School of Journalism. In addition to my journalism
board or commission: degree, I earned an undergraduate Environmental Studies
certificate through the Voinovich School of Leadership and
Public Affairs. For the certificate program, I took courses
relevant to the Catfish Creek Watershed Management Authority
position including: "Ecology in the 21st Century",
"Environmental Geography", "Environmental Law", "Water and
Pollution", and "Environmental Sociology". My capstone class
6•
for my journalism degree was"Environment and Science
Journalism". My capstone class for the environmental studies 1
certificate was"Climate Change Analysis". Completing this
array of courses created a foundation of environmental,
ecological3, and climate-based knowledge that I believe help J
qualify me for this position. During my time at Ohio University, I
was the 'Lifestyles and People' section editor for College Green
Magazine for two years, as well as a contributing writer. I was
also an Assistant Editor for three months at several
publications of the Water Environment Federation, including: I
"World Water", "World Water: Water Reuse & Desalination",
and "World Water: Stormwater Management". I also spent one {
summer as a research intern for EcoWatch.com. Since that
time, I have held several positions relevant to these studies: -
Six months with the nonprofit organization Rural Action (The
Plains, OH) as a communications intern/coordinator for the
Appalachian Ohio Zero Waste Initiative (25 to 30 hours a week)
-11 months with Winneshiek County Energy District/Green
Iowa AmeriCorps (Decorah, IA) as the Marketing Coordinator
(35 hours a week) -1 year(and currently ongoing)with the
Sisters of St. Francis (Dubuque, IA) as the Director of
Marketing & Public Relations (40 hours a week) I have
volunteered time with community environmental groups such
as the Friends of Decorah Parks, and the Decorah Tree Board.
The former was to remove garlic mustard from Decorah city
parks, and the latter was to plant trees on Decorah city
property. In Decorah I was a member of the Oneota Film
Festival board, and applied for grants and rights to screen
several environmentally-related documentaries to Decorah,
including: "River Sojourn", "Mysteries of the Driftless",
"Empowered: Power from the People", "Dirt! The Movie
"Switch: Discover the Future of Energy", "Bikes vs Cars", "The
Future of Energy: Lateral Power to the People". In Athens, as
an editor of College Green Magazine, I arranged for theatrical
screenings of"Trashed" and "People of a Feather". I am a
dues-paying member of the Society of Environmental
Journalists and the Sierra Club. In my free time, I occasionally
take massive open online courses from UW-Madison, including
"Changing Weather and Climate in the Great Lakes Region",
file:///C AJsers/tgleason/AppData/...
and "Forests and Humans: From the Midwest to Madagascar".
What contributions can I was born in Minnesota, I grew up in Southwest Wisconsin
you make or state (Lancaster& Belmont), lived in Athens OH from 2009 to 2014,
reason for applying? and moved to Iowa in 2014 where I have remained since. My
parents in the year 2000 bought a 70-acre lake called Lake Joy q
in Wisconsin, about 40 minutes north of Dubuque. While it's a
man-made lake, it has since grown to become an important
habitat for herons, frogs, and turtles. The lake also serves as a l
fish hatchery. I state all of this because I am from the Midwest,
born and raised, and care about our ecosystems and
environment here. Water has always been an important issue
for me, perhaps what I consider to be my number 1
environmental issue. The midwest is a critical juncture for water
supply in the U.S., first being home to the Great Lakes—one-
fifth of the world's total supply of fresh surface water and 95
percent of the U.S. supply of fresh surface water—and
second, being home to the mouth of the Mississippi River— 1
the world's fourth largest river. However, citizens so often focus
I,
on those big bodies of water such as the Great Lakes and k1
Mississippi River, that they forget the importance of the water
l
running through their own communities and backyards. Building
upon my Environmental Studies education, my family roots in jl
this area, my love of and concern for water, and my past work
volunteering for community environmental groups— I feel as G
though this position with the Catfish Creek Watershed
Management Authority is a culmination of my interests,
education, and experience. I believe I could contribute
thoughtful ideas and considered vision for maintaining,
conserving, and protecting this important 57 square mile
watershed. Clean water is critical for sustaining all life. j

good faith effort to appoint a qualified person to fill a vacancy on the commission for a period of
three (3) months but has been unable to make a compliant appointment, the city must utilize a
fair and unbiased method of selecting the best qualified applicants.
8-2-6: OATH:

C. To hold hearings upon any complaint made against a person, an employer, an
employment agency, a labor organization, or the employees or members thereof, to
subpoena witnesses and compel their attendance at such hearings, to administer oaths
and take the testimony of any person under oath and to compel such person, employer,
employment agency, labor organization, or employees or members thereof, to produce g
for examination any books and papers relating to any matter involved in such complaint.
The commission will issue subpoenas for witnesses in the same manner and for the
same purpose on behalf of the respondent upon the respondent's request. Such hearings
may be held by the commission, by any commissioner, or by any hearing examiner
appointed by the commission. If a witness either fails or refuses to obey a subpoena
issued by the commission, the commission may petition the district court having
jurisdiction for issuance of a subpoena and the court must, in a proper case, issue the

judgment of the commission to promote civil and respectful behavior which tends to
eliminate discrimination.
N

J. To receive, administer, dispense, and account for any restricted funds that may be
voluntarily contributed to the commission and any grants that may be awarded the
commission for furthering the purposes of this Title. No disbursements will be made of
any restricted funds without authority from the city council.
8-2-14: RECORDS TO BE PUBLIC; EXCEPTIONS:
All records of the commission are public except charges, complaints, reports of investigations,
statements, and other documents or records obtained in investigation of any charges.
8-2-15: CONFIDENTIALITY OF COMPLAINTS:
No member of the commission or its staff will disclose the filing of a charge, the information
gathered during the investigation, or the endeavors to eliminate such discriminatory or unfair
practice by conference, conciliation, or persuasion unless such disclosure is made in connection
with the conduct of such investigation or after the commission has held a public hearing upon a
complaint filed in connection with such charge. This Section does not prevent any complainant,
witness, or other person from publicizing the filing of a charge or complaint or the matter therein
complained of.
8-2-16: SUBPOENAS:

SUMMARY OF
MEDIACOM CABLE SERVICES FRANCHISE
PROPOSED RENEWAL AGREEMENT
April 13, 2005
r
1. Franchise Term
The proposed franchise term is fifteen (15) years. The franchise would be
for the delivery of cable services and would be nonexclusive.
i
2. Broadcast Basic Tier
y
While the size, contents, and pricing of a basic cable service package
cannot be a formal term of the agreement, Mediacom has represented to
the City that it will begin to offer a limited "broadcast basic" package with
21 channels. By federal law, these must include the broadcast TV
stations and the local PEG (Public, Educational and Government) access
channels.
3. Senior Citizen Discount
!I
Mediacom will maintain the current ten percent (10%) Senior Citizen
discount on the 72-channel Family Cable tier, which is currently priced at
$45.50 for most subscribers, $40.95 for seniors. There will be no discount
on the price of the limited "broadcast basic" package. A subscriber(head
of household) who reaches the age of 65 and submits an application to
Mediacom with proof of age and proof of at least one other utility in their
name at that address will qualify for the Senior Citizen discount.
4. Technology Updating Fund1
j
To keep the cable system as technologically current as possible,
Mediacom will contribute $300,000 per year in years one through twelve of
the franchise, (totaling $3.6 million), to an Updating Fund. This money l
can only be spent by joint agreement of the City and Mediacom.
i
5. System Upgrade
Within twenty-four (24) months of the signing of the agreement, Mediacom
will upgrade the electronics in their Dubuque cable system to increase the
total bandwidth (capacity)from the current 750 MHz to 860 MHz.
Mediacom would expend all or part of the first four years of contributions
to the Updating Fund (above) on this upgrade. l

l
w
6. Annexation
Consistent with the City's obligation to extend utilities, Mediacom would
have up to three years to provide cable television services throughout an !o
annexed area.
li
7. Digital vs. Analog
While this issue will be subject to significant FCC regulation; Mediacom
will be permitted to convert digital signals to analog to avoid making old
analog television.sets obsolete. They will also strive to pass digital and
high-definition (HDTV) signals to the greatest extent possible to
accommodate new television receivers designed for these signals.
Eventually, the entire television signal spectrum will evolve to digital
technology.
8. Institutional Fiber Network(I-Net)
Within the first 18 months of the agreement, Mediacom will construct a
new "dark fiber" Institutional Network. The term, "dark fiber" means that a
users, not Mediacom, provide and operate the end devices to light the
fiber. Separate from the subscriber network, this I-Net will interconnect 77 h
government, educational and not-for-profit facilities in Dubuque. The City
will be able to use the I-Net for any non-commercial purpose. Hospitals
and medical facility use of the I-Net will be restricted to community
emergency situations, and all other not-for-profit facilities, including
educational institutions, will be subject to restrictions on their use of the I-
Net for communications outside the network.
1
The City will possess an indefeasible right to use the fiber optic plant
dedicated to the ]-Net, and will also have the option to purchase the 1-Net
from Mediacom on terms stipulated in the franchise agreement.
i
9. ICN Links
Mediacom will continue to maintain and operate all existing links to the
Iowa Communications Network (ICN) at no charge, and the new fiber I-Net .
may be interconnected with the ICN, consistent with I-Net use
requirements.
10. Business Fiber Optic Infrastructure and Services
B
In addition to existing Mediacom fiber optic facilities, as the I-Net is
constructed, at least 6 additional new optical fibers will be installed along
its route for Mediacom to use in delivering commercial networking services
to business and industry. Mediacom will extend fiber in the right-of-way to
the curb within six months of a signed agreement by any business for a
large bandwidth service requiring fiber, at no additional cost to the
w
4
i'
i
business. The monthly charge for the particular communication service
w will be negotiated between that business and Mediacom. h
11. Industrial Parks
In addition to any existing Mediacom fiber and the new business fibers to
be installed along with the I-Net, Mediacom will extend designated fiber
links to Dubuque Technology Park and Dubuque Industrial Center West
within two years of the agreement. h
12. Free Service To Certain Facilities
Ij
Upon request of the City, Mediacom will provide one activated cable
service drop and Basic Cable Service to each school and each facility j
owned or leased by the City at no charge. Each institution may further Ij
distribute the service within the facility at its own expense for uses
consistent with the institution's mission.
i
r
Also, Mediacom will provide one 200-ft. (maximum) drop and maintain one
connection for Internet access to one computer terminal in each school or
library within the franchise area at no charge. Any additional costs will be
borne by the school or library for a time and materials charge.
13. Trenching
i
When either the City or Mediacom is trenching in the public right-of-way,
the other party would have the right to install its own conduit at the same
r
time. However, if the City ever establishes a municipal communications
utility that competes with Mediacom, the City would have to reimburse
Mediacom on a pro-rata basis for its portion of the trenching costs for any
cable the City previously installed in Mediacom trenches.
14. Importing Broadcast Television Signals
All broadcast television station signals will be imported into the Dubuque
system by optical fiber, resolving a 50-year-old reception problem. t
d
15. Universal Service
I
I,
Mediacom is required to extend residential-type cable service to all
businesses and residences within the City.
i
16. TDD/TTY 9
i
Mediacom will direct hearing-impaired callers to Answer Iowa, a state-
wide service, for assistance. The company will clearly advertise the
availability of the Answer-Iowa service.
i
e
i
i
y
i
17. Emergency Alert
Unless preempted by the Federal Emergency Alert System, the City would
continue to have the ability to issue local alerts, subject to regulation by
the FCC.
18. Franchise Fee
i
Mediacom will pay a franchise fee of five percent (5%)of gross revenues
on cable services. If any action of law expands the base upon which
franchise fees may be collected, Mediacom will automatically expand its
computation and payment of franchise fees in Dubuque accordingly. it
Mediacom will continue to treat the franchise fee on subscriber monthly
bills as a cost recovered from within the rates for services, and will not list j
the franchise fee as an additional charge in the manner they list sales G
taxes.
19. PEG (Public, Educational, Government)Access Channels
Mediacom will continue to provide on its subscriber network two public
access television channels, one shared educational access channel, and
one government access channel, as is currently offered. In addition,
Mediacom will provide a second educational access channel dedicated to
24-hour, 7-day programming by the Dubuque Community Schools. On or
before December 31, 2006, Mediacom will also make PEG Access
programming available to digital subscribers through its Video On Demand
service. At such future time as Mediacom carries local broadcast stations h
in high definition (HDTV), any PEG Access channel will also be
transmitted in HDTV format upon request of the entity managing the
channel
Throughout the term of the franchise, Mediacom will continue to provide
studio production facilities, production assistance, editing, scheduling and
playback, equipment maintenance, training, outreach, management and i.
administrative support for users of the public access channels at the same
level as was provided as of January 1, 2005.
p
20. Capital Grants for PEG Access Equipment and Facilities
l
Separate from the franchise fee, Mediacom will deposit 1.75% of gross
revenues in an interest-bearing account from which the City may draw for
capital support for PEG and I-Net use, in its sole discretion, provided that
the City will not use more than 50% for I-Net support.
21. Charitable.Foundation
While not included in the franchise agreement, Mediacom will form a
charitable foundation to which the company will contribute .25% of cable

service gross revenues (estimated at $25,000 per year)to distribute in the
community. Initially, the fund will focus upon arts and cultural affairs. The
board of the foundation will include Mediacom employees and members of
the City's Cultural Affairs Advisory Commission.
I;
22. Remedies I
Instead of moving directly to termination of the franchise as the only
penalty for a violation of the franchise agreement, the new franchise
provides incremental steps and monetary penalties for different types of
violations.
23. Right-of-Way Ordinance
Mediacom's use of the rights of way will be the subject of a separate
ordinance based upon one devised by the Iowa Municipal Attorneys
Association, with Corporation Counsel Barry Lindahl participating.
Although this ordinance will initially apply only to Mediacom, the goal will
be to eventually apply the right of way ordinance to all utilities.

69.16A Gender balance.
1. All appointive boards, commissions, committees, and councils of the state
established by the Code, if not otherwise provided by law, shall be gender balanced.
No person shall be appointed or reappointed to any board, commission, committee, or
council established by the Code if that appointment or reappointment would cause the
number of members of the board, commission, committee, or council of one gender to
be greater than one—half the membership of the board, commission, committee, or
council plus one if the board, commission, committee, or council is composed of an odd
number of members. If the board, commission, committee, or council is composed of
an even number of members, not more than one—half of the membership shall be of one
gender. If there are multiple appointing authorities for a board, commission, committee,
or council, they shall consult each other to avoid a violation of this section.
2. All appointive boards, commissions, committees, and councils of a political
subdivision of the state that are established by the Code, if not otherwise provided by
law, shall be gender balanced as provided by subsection 1 unless the political
subdivision has made a good faith effort to appoint a qualified person to fill a vacancy
on a board, commission, committee, or council in compliance with subsection 1 for a
period of three months but has been unable to make a compliant appointment. In
complying with the requirements of this subsection, political subdivisions shall utilize a
fair and unbiased method of selecting the best qualified applicants. This subsection
shall not prohibit an individual whose term expires prior to January 1, 2012, from being
reappointed even though the reappointment continues an inequity in gender balance.
